Many GP practices in Sheffield have a Community Referral (Social Prescribing) service. Community Referral workers provide free, confidential one-to-one support, helping people identify what matters to them and connecting them with local services, groups and activities that may improve their health and wellbeing.

How to access Community Referral
- Ask your GP practice if they offer a Community Referral (Social Prescribing) service.
- If they don’t, you can contact Sheffield’s city-wide Community Support Workers, who can provide advice and help you access local support.
